Assessment Task 3: Observation
Student Assessment Instructions
- Your assessor will explain the arrangements for this assessment, including when it will occur and how the observation will be conducted.
- You must satisfactorily complete all sections of the assessment task.
- Ask your assessor to clarify any aspect you are unsure about in this assessment task.
- You may be asked to explain some parts of the demonstration if the assessor needs any clarification.
- You may need to work in groups via Zoom.
- Your trainer will provide you and all other participants with a Zoom link for you to undertake this assessment/observation task.
Required Resources
- You will need to have access to relevant legislation, standards and codes, industry practices, student book resources computer and internet access.
- You may need access to organisational documents and deliverables from previous tasks in the assessment.
Evidence
- You must submit the completed assessment cover sheet and other evidence as outlined in the observation instructions, by online submission within the allocated time.
- Ensure you keep a copy of your submitted Assessments submitted without completed cover sheet will not be accepted.
When and where will this assessment take place?
- This assessment will be undertaken in a simulated environment/in a classroom/off site via a cloud-based video conferencing application) This is to be discussed with the assessor.
- Your assessor will provide you with the due date, which can be recorded in the assessment plan.
- Your assessor may also specify the length of time allowed and the expected word count, if applicable.
What if the assessment is not suitable?
- If you are unable to provide sufficient evidence within the observation assessment method, your assessor may be able to provide you with an alternative method. Discuss this with your assessor.
What happens if your answers are not satisfactory?
- If any of the observation assessments are not satisfactory, your assessor will provide you with You may need to resubmit some or all the assessment task. Your assessor will explain the details for your reassessment.
- In most cases, you will be allowed a maximum of three attempts.

Part A Role Plays
Instructions
You are to complete three role plays based on the case studies provided below. Each role play should take a maximum of 10 -12 minutes. You will need a person (over the age of 18 years old) to play the role of the client, based on the client background information provided.
Your assessor will observe the role plays and use the Criteria Checklist to confirm your ability to demonstrate the required skills in each of the three role plays.
Case Study One
Introduction
You are employed by an approved provider of aged care services in the role of Care Coordinator. The service provides flexible and personalised services to support older people to live at home independently and safely. Mrs Ali Chang has been referred to you by the Aged Care Assessment Team (ACAT) after they undertook a functional-based assessment with her to determine hthe level of assistance Mrs Chang requires with her Activities of Daily Living (ADLs).
You have now scheduled to meet with Mrs Chang to discuss the ACAT assessment outcomes and to identify what services and support your organisation can provide.
Client Background
Mrs Chang is a 84 year old woman of Chinese descent who has lived in Australia for over 50 years. Mrs Chang does not need an interpreter as she speaks, reads and writes English fluently. She lives alone in the large family home after her husband passed away 6 months ago . Her only child, a daughter, lives over 200kms away and visits monthly.
Mrs Chang’s mobility has been slowly declining and she now needs support with personal care including showering and dressing and domestic assistance including hoovering, cleaning and hanging out washing. Mrs Chang has the goal of staying in her own home for as long as possible.
During the assessment Mrs Chang, mentions that she misses her husband terribly. She is now sleeping for long periods during the day and has lost interest in things that she usually enjoys. Mrs Chang does not drive and since her husband passed away is no longer able to participate in her social activities.
Case Study Two
Introduction
You are employed by a community health service in the role of Alcohol and Other Drugs Intake Officer. The service provides alcohol and other drug counselling and group therapy with the aim of
helping people manage their drug and alcohol addictions. Patrick has been referred to your service by a homeless shelter.
You have now scheduled to meet with Patrick to discuss what services and support your organisation can provide.
Client Background
Patrick is a 43 year old man who has lived in Australia all of his life. He has a history of alcohol abuse and as a consequence his marriage resulted in divorce 5 years ago and he has not had any contact with his two school aged children since.
Unable to keep a permanent job due to his alcohol abuse Patrick is unemployed and has no permanent accommodation. Due to the effects of drinking he struggles to maintain his health which has resulted in poor nutrition, high blood pressure, weakening of his immune system and, at times, blackouts and memory problems.
Patrick, during his intake assessment said that his priorities are to find stop drinking, find stable accommodation, improve his health and reconnect with his children.
Case Study Three
Introduction
You are employed by an approved provider of disability support services in the role of Group Home Manager. The service provides personalised supported independent living solution for people with a disability. Julie has recently moved into the Group Home and your role is to help her settle into the Group Home and support her in achieving her goals.
You have now scheduled to meet with Julie to discuss what support your organisation can provide to assist Julie to reach her goals.
Client Background
Julie is a 27 year old woman who has until recently, lived with her parents in the family home. Julie was born with Down syndrome, a genetic condition that causes mild to serious physical and developmental problems. Julie’s ability to think, reason, understand, and be social has been affected by her condition.
Her parents are concerned that she will not be able to manage living independently. This has caused some conflict between the family and Julie has not seen her parents since making the decision to move out of home. This has made Julie very upset and she hopes that their relationships will get better.
Julie, during her intake assessment said that her priorities are to mend her relationship with her parents, find paid part-time employment and develop her social skills and meet new friends.
