Description

This assessment requires the student to answer set questions using a template (available in Canvas) on a specific case study of a forensic patient who is to be released from custody. The activity is designed to facilitate and develop your professional writing skills and develop your ability to complete a comprehensive discharge report.

	INSTRUCTIONS: Mr X is to be released from custody on a stated date after review by the Mental Health Review Tribunal. He is to be released with conditions for the provision of structured mental health care from the community mental health team. You are responsible for writing a discharge report for Mr X using the case study information (available in Canvas) so that a management plan can be developed that is appropriate for his cultural, mental health, and social needs. This assessment requires you to answer the following questions using the template provided in Canvas. 1. What legal, biological, psychological, environmental, and social-occupational treatments are to be considered and how can these be incorporated into a community managed treatment plan? 2. What are the anticipated problems that may arise within the community that require addressing and how should Mr X be managed? 3. Evaluate the protective factors that Mr X has and what is his risk of offending in a violent manner in the future? How should this risk be assessed? 4. Provide a management plan to manage the identified risks and Mr X’s mental health. Reference List Note: Your work must provide appropriate high quality contemporary references to support your decision making. Ensure your opinion is supported by the literature to avoid personal bias.
