Assessment Task
In this assessment, you will research and write a report on the health condition of your chosen case study. Your report will address a number of key points outlined in the instructions. This report (Assessment 2) will provide the justification for an education presentation that you will record in Assessment 3.

Context
The principles and philosophy of primary healthcare, health promotion and community development practice will be called upon in exploring the health of individuals and communities. Health professionals need to develop their capacity for empathy and understanding of the lived experience of vulnerable population groups as well as pathways for treatment and possible barriers to accessing these.
Understanding these principles and philosophies is critical in developing appropriate community nursing care. Without understanding the fundamentals underpinning any profession, it can be difficult to appreciate why procedures are delivered in the way they are.
In Australia, eight common chronic conditions contribute to
- 61% of the burden of disease
- 37% of hospitalisations
- 87% of deaths (based on underlying or associated causes of death)
The eight conditions are arthritis, asthma, back pain, cancer, cardiovascular disease, chronic obstructive pulmonary disease, diabetes, and mental health conditions. (AIHW, 2018)
In addition, The National Strategic Framework for Chronic Conditions has identified nine Priority Target Populations, those that are negatively impacted by chronic conditions more than the general population. This is demonstrated by a higher prevalence of chronic conditions and a greater burden of disease in these populations resulting in inequitable health outcomes.
Primary health care is essential for improving health outcomes now and in the future.

- Research the health condition of your chosen case study and write a report that addresses the questions This includes identifying their health education needs. Your report will justify the need for an education presentation tailored to meet the needs of your chosen character in this assessment and conduct the education session as a digital presentation in Assessment 3
- Your report should include the following key points:
- Outline the target health condition of your case study character and outline why it is/ is not one of Australia’s eight common chronic conditions?
- Identify and explain the risk factors for developing the stated health condition?
- Identify and explain the long-term consequences of the condition?
- What are the health issues/risks specific to your character?
- Identify and analyse 3-4 social determinants of health affecting your chosen
